Independent Study Requests

  1. Go to:  creeksidecharterschool.powerschool.com/public

  2. Login with the same login you used to sign the Master Agreement or other school forms

  3. Click Forms on the left-side menu

  4. Click on Independent Study Request in the “As Needed Forms” section

Parent In-School Volunteering

All parents who will be interacting with children on-site will need to be fingerprinted by the Burton Creek Sheriff’s office, the UPS Store in Truckee or the Office Boss in Truckee.  Please fill out and bring with you the LiveScan form located on ParentSquare under Explore > Resource Hub.  The process after your appointment can take up to two weeks so please plan ahead if you would like to volunteer.

Parent Chaperones for Field Trips

All parents who will be interacting with children during a field trip will need the following: 

  1. to be fingerprinted by the Burton Creek Sheriff’s office, the UPS Store in Truckee or the Office Boss in Truckee.  Please fill out and bring with you the LiveScan form located on ParentSquare under Explore > Resource Hub.  The process after your appointment can take up to two weeks so please plan ahead if you would like to drive.

  2. to bring in a copy of their current Drivers License and automobile insurance to the office.

  3. to read, sign and print the Chaperone Code of Conduct located on ParentSquare under Explore > Resource Hub..  Please bring the printed copy to your child's teacher.

Administration of Medication at School

If your child needs prescribed medication administered at school, please fill out the Authorization to Administer Medication by School Personnel located on ParentSquare under Explore > Resource Hub with your child's physician and turn it in to the office.  Children cannot have medication of any kind at school without prior authorization from their doctor and the school nurse.

Class Lists

Class lists are located on ParentSquare under Explore > Directory.  Please note that if a child's parent has opted out of having their information in the directory, you will not see it there.
