Independent Study Requests

This form is located on ParentSquare.

1. Log into your ParentSquare account.

2. Go to your student’s class feed.

3. Find the Independent Study Request form pinned at the top of the feed.

4. Click on it to complete the form.

Parents must submit a form for each student.

Parent In-School Volunteering and Field Trip 

All parents who will be interacting with children on-site will need to be fingerprinted. You can complete this process at Truckee Fingerprinting or the Truckee Police Department. Please fill out and bring with you the LiveScan form located on ParentSquare under Explore > Resource Hub. The processing time after your appointment can take up to two weeks, so please plan ahead if you would like to volunteer or drive on field trips.

Parent Chaperones for Field Trips

Parent volunteers and drivers are required to complete fingerprinting (see instructions above) to ensure the safety of our students. In addition, all parent volunteers and drivers must complete the Parent Chaperone/Driver Form, which is located in the Resource Hub on ParentSquare. Once submitted, your information will be valid for the entire year unless your license expires or you change insurance providers. Thank you for your cooperation!

Administration of Medication at School

If your child needs prescribed medication administered at school, please fill out the Authorization to Administer Medication by School Personnel located on ParentSquare under Explore > Resource Hub with your child's physician and turn it in to the office.  Children cannot have medication of any kind at school without prior authorization from their doctor and the school nurse.

Class Lists

Class lists are located on ParentSquare under Explore > Directory.  Please note that if a child's parent has opted out of having their information in the directory, you will not see it there.
